Billing Policies:
We are preferred providers for many PPO insurance carriers and Medicare. These are United Healthcare, Blue Shield, Cigna, Blue Cross, Aetna, PHCS and Health Net. However, all of the above insurance carriers have many different types of plans within their network and we may not contracted with all of these subsidiary plans. To verify our participation with your specific insurance policy, please contact your insurance company directly and provide them with our Tax ID number. We can be found listed as: UCSF Physician Group, with Tax ID: 47-5502470.
It is very important that you bring your insurance card with you on your first appointment. For returning patients with any changes to your policy or coverage, please be sure to inform us and bring your updated card with you. If we cannot verify coverage and if you wish to be seen anyway, you will be held responsible for the cost of the visit at that time.
Payment for all cosmetic services including cosmetic consultations is required at the time of service. We accept all forms of credit cards.
We ask that you please give us at least 24 hours’ notice if you cannot make your appointment so that we can fit someone else in. For Monday patients, please call as early as you can the Friday before your appointment. We charge a $75 dollar no-show fee if you miss your scheduled visit and do not notify us one full business day prior to your scheduled appointment.
You will get a courtesy text reminder of your appointment approximately two days prior to your appointment.
